How to Print out a CAP Form 101 Card
The CAP Form 101 Card is a pocket sized Specialty Qualification Card that you can carry in your pocket.
To print out a copy of the 101 card, do the following:
Go to the eServices national website at https://www.capnhq.gov/CAP.eServices.Web/Default.aspx
Enter CAPID and Password.
Click on My Operations Qualifications (on the left side).
Click on 101 card (at the middle left).
The resulting card should have your picture, a bar code, and a list of your specialties and expiration dates.
If you are in training, your Specialty is preceded by an asterisk (e.g. *MS).
To Print a copy of the card, click on: “Click here to print”.
Cut the printout to wallet size and carry it with you when you are training and especially when you report to
Mission Base.
Note: You can check all of your qualifications two ways:
1. click the “View Qualifications” field under your name at the top.
2. click “Single Person” under “Emergency Services” on the Operations Qualifications screen.
